For Childhood Strabismus (up to 16 years of age):
Referrals are accepted from eye care practitioners (ophthalmologists, optometrists). Referrals must be faxed or mailed to the eye clinic strabismus co-ordinator at The Hospital for Sick Children using the hospital's standard form.
The referring doctor can either submit the referral on-line at: www.sickkids.ca/referralsystem
or
fax the required information to the eye clinic on-line referral system (ARMS), at 416-813-6261. Copies of any other relevant information can be faxed at the same time.
Alternatively, if there is a problem with the on-line fax system, the form and accompanying information can be faxed to Dr. Kraft’s office at 416-813-5159, or mailed to his Sick Children’s office address below. His co-ordinator at Sick Kids can be contacted by phone at 416-813-8375.
The patient or patient's family is then contacted with an appointment date. In almost all cases the appointment is co-ordinated with an orthoptic (eye muscle testing) visit prior to seeing the doctor.
Due to long wait lists for appointments, in appropriate cases alternative specialists may be recommended.
For Adult Strabismus (age 16 years or older):
Referrals are accepted from eye care practitioners (ophthalmologists and optometrists). Referrals can be made to Dr. Kraft’s office fax number, at 416-813-5159, or by mail to Dr. Kraft’s Sick Children’s office below.
The visits take place at the Eye Clinic at the Toronto Western Hospital, 399 Bathurst St., Toronto, ON. It is located on the 6th floor in the Ophthalmology Technical Services area (see below).
Dr. Kraft’s secretary will contact the patient with the date of the consultation and notify the referring doctor’s office of the time of the appointment. In almost all cases the appointment is co-ordinated with an orthoptic (eye muscle testing) visit prior to seeing the doctor.
For Botulinum Treatment (Botox, Xeomin) for Blepharospasm, Hemifacial Spasm, Facial Tics, or for Corneal Healing Problems:
Referrals are accepted from ophthalmologists and neurologists. A copy of the complete eye or neurologic assessment should be faxed or sent to Dr. Kraft at his office.
Patients are generally booked for consultation and possible treatment for the same day, usually on a Tuesday morning. Waiting time is usually less than one month.

Patients in Thunder Bay and Area (paediatric ophthalmology and strabismus only):
Dr. Kraft visits Thunder Bay once or twice per year to run clinics in paediatric ophthalmology and strabismus and to perform strabismus surgery. Patients are referred by the ophthalmologists in Thunder Bay.
Currently, the clinics are held at the Port Arthur Clinic. Referrals are to be sent in writing with all relevant information to the eye clinic co-ordinators at the Port Arthur Clinic.
